The Relocation of Programs - Teacher Survey has been designed to determine the impact that relocating programs has on teaching and learning within a specific school.
Reflection:
Though I have created surveys before they were often informal and for my sole use in order to provide me with immediate feedback from the students regarding my practice. As a result they were often done quickly and with not much more than minimal thought going towards the wording and layout.
However, when creating this survey much more time and care was taken to ensure that the intent and purpose of the survey was reflected in the wording and layout of the questions. Since teachers are the audience for this particular survey it was determined that a more advanced level of language could be used, including professional jargon if needed. However, being cognisant and respectful of teacher's time the survey was paired down and created in such a way to provide the required data in the simplest way possible.
The original survey was piloted by three people (a teacher, a friend, and someone who has no relationship, directly or indirectly, to the school).
Original Survey:
Google Forms was used to create the survey because this was a tool that the school had access to and was already using so the user should be familiar with the format of the survey. Furthermore, the school board as a Google Education account and therefore can control access to the survey and information.
Google Forms also provides you the option to generate a print version of the survey if an electronic version is deemed to not be the best method to administer the survey. The majority of the question responses would be manditory though this has been removed for the purpose of this assignment so that the entire survey can be viewed without having to complete it.

Teachers refer to their programs as classes so it was suggested to change the wording of programs to classes. Also, since the number of programs that are taught vary the singular and plural forms need to be used. Provide survey support information. Even though the evaluation was specific to the guitar program it made sense to look at program location in general. It was also noted that it did not single out a specific person, group, or program as that could create resentment amongst the staff at the school. General feedback was also provided with respect to wording, spelling, and grammar. A progress bar was added to allow the audience to see where in the survey they were. It was felt that this would encourage participants to complete the survey as they could see their progress.
Background Information: Some felt that the teacher should include their name so that they can be identified if further clarification is needed. However, this was not deemed necessarily for the validity of the data. If the administrators decide that this information is required then it will be added. It can be self populated by requiring the teachers to log in to their board approved Google account.
Ideal Class Location: The suggestion was made to have the teachers provide insight and reflect upon what makes an ideal class location. This would provide valuable data to the evaluators when making recommendations about how to ensure that classes are located in ideal areas in the future. Furthermore, the teachers will then be thinking about this as they reflect on the current location of classes within the school. Environmental factors (sound, light, and physical space) as well as location with respect to proximity were discussed. For the purpose of this assignment only a sample is provided. However, through further testing and with greater consultation of the client this would be filtered to a more appropriate and beneficial list.
Current Class Locations: Concerns were brought up around the word "conducive" however, as stated earlier the audience is academic and therefore the wording should not pose a problem for the teachers. Questions, 10,12,14, & 16 all ask the audience to go to another question based on the answer which some found confusing. Because of the limitations of Google Forms I was unable to change this so that question was hidden based on how the audience responded. The solution to this would be to create new pages though I felt that it would be more confusing for the audience to have to click continue to go on to a new page each time.
